By signing below, I acknowledge and agree that:

I have provided complete and accurate information about my child's medical history, allergies, and special needs.
I authorize Little Guests Event Childcare to obtain emergency medical care for my child if I cannot be reached.
I understand that normal risks are associated with children's play and activities.
I release and hold harmless Little Guests Event Childcare, its owners, and staff from any liability for injuries due to undisclosed medical conditions or inaccurate information.
I authorize only the listed individuals to pick up my child.
